Written notice is required by Elite once a catastrophic claim, such as a motor vehicle accident, transplant, premature birth, etc. has occurred or once the claim has exceeded 50% of the Specific deductible. Our claim forms are designed after the Self Insurance Institute of America, Inc. (SIIA) standard.
